The car was travelling up a steep road in rugged terrain up Longley Road, Porepunkah, at the foot of Mount Buffalo on Saturday at about 8.20am.
The car then lost grip, flipped and rolled for 400 metres down the embankment.
An ambulance spokesman said the rough terrain caused long delays in reaching the injured - with the most injured man, 18, taking nearly five hours to reach The Alfred. The terrain was so tough that helicopters could not reach the teenagers at the scene of the accident and the injured had to be taken to a more accessible area by road.
Three ambulance road crews and two helicopters were joined by State Emergency Services volunteers to get the injured out of the area.
The 18-year-old had head and chest injuries and multiple fractures. He was in a critical condition and was the first to be air-lifted.
A 17-year-old had pelvic and arm injuries and was taken to the Royal Melbourne Hospital by helicopter in a serious but stable condition.
A third male, in his late teens, was taken to a local hospital by road ambulance with chest injuries, cuts and abrasions.
