The 34-year-old polled 67 votes to edge out captain Shane Gaston (65) and co-coach Sam Carpenter (55) for the Bob Rose Medal.
“Both Sean and ‘Gatto’ had really good years for us,” Carpenter said.
“Sean’s showed age is only a number on your birth certificate because he’s been super for us.
“He gets a lot of footy off the half-back flank.
“We wish we had two of him because we need him on the ball as well.”
Carpenter said O’Keeffe had joined most of the Hawks’ list in re-signing for 2017.
