A Toyota Camry and a Toyota Hilux collided head-on on Wangaratta-Yarrawonga Road south of Killawarra around 5.30pm.
The teenage boy, a passenger of the Camry, was airlifted to Melbourne with head and leg injuries.
As of Saturday evening, he remained in a critical condition.
Ambulance Victoria spokesman John Mullen said two others were taken to Wangaratta Hospital.
“We transported a man in his 20s with pelvic injuries, in a serious but stable condition … we also conveyed a woman in her late teens who had facial injuries,” he said.
Detective Senior Constable Phil Frith said the road re-opened at 5am on Saturday but the investigation was ongoing.
“We’ll be looking at all the factors as usual – speed, alcohol, drugs or distractions will all form parts of the investigation,” he said.
“We’ve identified the relevant witnesses.
“It’s a call for people to take care at this time of year – we need people to take care on our roads.”
