Emergency services were called to the popular tourist spot on Thursday after reports of a fall.
A group of young people had been on the middle section of the rock formation when the man slipped.
He slid down the falls and suffered serious head injuries.
The incident led to a rescue operation involving the SES, police, paramedics and Parks Victoria.
The man was stretchered out of the area before being flown from Porepunkah to a hospital in Melbourne.
There are fences and warning signs in the area but paramedics regularly attend incidents at the falls.
The signs were installed after previous incidents.
The site has a steep vertical drop of about 50 metres near rock pools, which people regularly swim in.
The rocks are dangerous with signs warning of the risk of severe injury.
Green moss grows under the water, which makes the surface slippery.
One death was recorded at the falls in 1931, but dozens have been injured.
In one of the more serious incidents, a tourist was submerged underwater for one minute after sliding down a waterfall, hitting his head and knocking himself out in December 2012.
A Melbourne man slid down the waterfall the following month, with police describing his injuries as being similar to a car crash.
Parks Victoria Mount Buffalo ranger Julien Atherstone has previously told The Border Mail it was hard to stop people from entering the area.
“Signage only stops the sensible and those wanting to hear the message,” he said.
“It’s a dynamic and rugged park and we can’t sign every risk.”
