Police were called to the incident at Boggy Creek Road in Moyhu about 7.20am on Sunday.
A Myrrhee woman had been transporting the horse when the trailer hit a deep culvert and flipped, landing upside down against a tree.
The horse was trapped upside down and was highly distressed, but there was little that could be done until a vet was called out.
The animal was asphyxiating itself due to its weight and its breathing became fainter.
The horse was eventually sedated, which allowed the SES to cut it out.
Leading Senior Constable Marielle Porter said the horse had only suffered a few small cuts.
“It’s a wonderful outcome,” she said.
“The vet checked him out and doesn’t think there are any major problems.”
Two teenagers also escaped injury after a four-wheel-drive rollover on the Bandiana Link on Sunday.
