The woman was airlifted to Melbourne from the scene with life-threatening injuries, while the teenager was in a serious, but stable condition.
He and three others were transported to Wangaratta Hospital.
The other three victims, including a four-year-old boy, suffered minor injuries.
The crash occurred on the Great Alpine Road when a four-wheel-drive, which was stationary waiting to turn right into the Tarra Tender Cuts butcher, was rear-ended by a Ford Territory, pushing it into the path of an oncoming Honda CRV.
Police do not believe alcohol or drugs contributed to the crash, but are investigating the possibility of driver inattention.
Acting Sergeant Patrick Murray urged drivers to take care, given the collision occurred on the eve of a four-day weekend in Victoria.
“This was a head-on impact, the car that was stationary was rear-ended, causing them to be shunted into the path of an oncoming car,” he said.
“We need people to take care – it’s the start of a four-day weekend, five days for some people.
“Statewide, Operation Furlong means every police car that is available will be on the road, every police car is an alcohol and drug testing unit.
“If you’re travelling for three hours, give yourself four, so you can stop and rest.
“If you need to go a bit slower and be more attentive, please do so – we want you to get home safely.”
The Great Alpine Road between Wangaratta and Tarrawingee was closed for several hours after the crash.
