Bradley Jordan Soderman was highly erratic on November 18 last year.
He went to his parents' home in Wodonga in a rage and threatened to slit their throats.
Soderman swore at his parents, and held the knife to his mother's throat after being asked to leave the property.
He smashed his dad's car and said he would "burn the house down while they were in it" if they called police.
He went to the Wodonga emergency department the same day and was handed medication, which he threw back.
Soderman smashed his elbow through plaster and was highly agitated with mental health workers, calling them "f---ing ---s".
He tried to spit on one worker and was restrained by security guards and other staff who were concerned for their safety.
He continued to be aggressive and later told police he had acted "out of pure frustration".
The mental health worker he targeted "can go f--- himself", Soderman said.
He was later arrested at the Wimmera hospital in Horsham on January 12, which he had attended after using drugs.
The Wodonga court on Thursday heard Soderman had been in custody since the arrest.
"It's a sad story your honour," lawyer Sally Wilson said, noting her client had once been a promising footballer.
He had stopped playing sport due to injury, lost his job and got into drugs.
Magistrate Ian Watkins said he would sentence Soderman next week but wants him to be supervised after he's released.
