It was a case of deja vu for the Cats who lost to the Hawks in the preliminary final last season.
Defender Ellen Cook starred for the Hawks in the win, while the efforts of Rebecca Evans in goals and Abby O'Brien in the midcourt didn't go unnoticed.
"Ellen was amazing," Kiewa Sandy-Creek coach Kath Evans said.
"Some of the intercepts she landed certainly justified her position as goal keeper in the team of the year."
The Hawks have now earned another crack at premiership glory after going down to Tallangatta in the grand final last season.

They will now face Thurgoona who finished the home and away season minor premiers.
"We set ourselves this goal a while back, so to make it is really rewarding," Evans said.
"Everyone has put a lot of hard work in throughout the year, so we're very excited.
"We played Thurgoona in the very first round and it's been a while since we've played them.
"They play some really nice netball so we'll just have to be good with our defensive pressure and capitalise on opportunities."
Saturday marked the end of Rutherglen's run through finals with the side putting up a good fight in the four goal loss.
Lily Buckingham shot 17 goals for the Cats, while Haylee Penny notched up 24 for the Hawks.
Rutherglen finished the regular season in second position.
