The Tigers were triumphant in the seniors and fourths, while the thirds went down by less than a kick.
Osborne won three flags after the 16 and under netball were also successful.
Culcairn is the only other club to achieve the feat in 1993 and won the seniors and the reserves.
The fourths wasn't recognised as an official competition until the following season.
Tigers president Jason Gooden said it was a proud moment for the club.
"I'm just so proud of all the players," Gooden said.
"It's a credit to the coaches and the players who put in all the hard work on the track throughout the year.
"History says it's hard to do with Culcairn the only other club to achieve the feat.
"In 2004 and 2005 we had three sides make the grand final but never four.
"In 2004 we won the reserves while in 2005 we won the seniors, reserves and thirds."

Despite Osborne and Brock-Burrum winning seven of the past eight flags combined, this season was the first time the pair had met in finals since the Saints merged in 2006.
The Saints won the second semi-final by 16 points before the Tigers turned-the-tables in the decider.
A talking point during the finals series was how difficult it was to read the numbers of the Osborne players.
Gooden said the club would endeavour to rectify the problem before next season.
"We did get a bit of feedback that the player numbers were hard to read," he said.
"We ordered the jumpers online, the numbers looked easy to read on the computer but it was a different story when we got them.
"We have to mindful of jumper clashes with some of the other clubs because there is a lot of black in the league.
"Brock-Burrum have a black back with white numbers, while the Billabong Crows have a black back with yellow numbers.
"We also have a bit of a jumper clash when we play Holbrook.
"It's not an issue for our supporters because they know the players but it is for people who don't."
Osborne will celebrate its 50-year anniversary in the Hume league next season.
Gooden said the Tigers were close to announcing a replacement for coach Matt Rava.
