Curtis backed up last year's victory by polling 146 votes at the SS& A Club on Friday night, with Martin Brennan runner-up on 131.
Shaun Mannagh (123), Tom Hargreave (80) and Nick Meredith (75) also polled well for the reigning premier.
Curtis joins Kade Stevens (2006 and 2007), Neville Shaw (1992 and 1993) and Bruce Stewart (1982 and 1983) as the only players to win successive awards.
Recruited from Ainslie, the long-kicking Curtis is the first premiership coach to win a best and fairest at Lavington since Jeff Cassidy in 1986.
Hayden Singe was a runaway winner in the reserves with 107 votes.
He finished well clear of Jim Oeser (70) and Sam Herzich (61).
In the thirds, Darcy Smith topped the count on 91 votes with Lachie O'Connor (82) and Alex Bennett (81) in hot pursuit.
Champion defender Sarah Meredith confirmed her status as one of Lavington's greatest netballers by winning the A grade best and fairest award.
Meredith polled 50 votes with Sarah Senini runner-up on 36 and Maggie Young third on 33.
Hailey Butler was victorious in B grade with 59 votes, with Sally Kapay runner-up on 32.
In C grade, Ovens and Murray best and fairest Grace Hay completed the double by polling 61 votes which was 27 clear of Hannah Barton.
Audrey Barker won the under-16 best and fairest from Olivia Sanson.
Lavington president Peter Barwick, long-time committeeman Graham Browne and 200-game netballer Erin Haberecht were awarded life memberships.
Browne was also named Club Person of the Year.
About 180 people attended the presentation night with John Hunt being honoured for breaking the league games record.
He played his 417th match in the Panthers' preliminary final win over Myrtleford at John Flower Oval.
Meanwhile, veteran on-baller Sam Carpenter has taken out Wangaratta Rovers' best and fairest from Nathan Cooper.
Nick Henderson was successful in the reserves and Alessandro Belci won the under-18s.
Ashlee Martin won the A-grade netball with Jenna McLeod runner-up.
