Shields, who will join Wodonga Raiders next season with teammates Tim Hallinan and Tim Haines, polled 182 votes to run out a comfortable winner.
Dane Hallinan (125), Tim Hallinan (78), Adam Prior (64) and Ben Gould (63) made up the top five.
Shields has finished runner-up in the past three Azzi medals and will leave the club as one of its best players of the past 20 years.
Culcairn is in the market for a new coach next season with Josh Hillary stepping down from the helm after two seasons.
In the reserves, Paul Steward polled 93 votes to win from Jesse Kent and Richard Wood on 88.
In the netball, Meg Smith won the A grade with Georgia Pitson runner-up.
Abby Austin, Tess McGrath and Samantha Erwin were successful in B, C and C reserve.
