A contractor at the quarry, operated by Burgess Earthmoving at Ettamogah, had estimated a blast exclusion zone of up to 500 metres was required for the incident.
But a preliminary investigation report released in January last year found members of the public had been between 300 and 340 metres from the blast site.
The regulator found seven people had been at risk of being struck by flyrock.
The late reporting of the incident was also under investigation.
The overburden blast was carried out by Hamiltons Blasting Services following preparation work and was caught on video.
The footage was supplied to the regulator.
A spokesman for the regulator this week said a final report was due to be released at the end of July - 22 months after the matter was reported.
It had been suggested the final report may have been released as early as October last year.
It's unclear what has caused the delays.
A 53-year-old driver had been stuck in the vehicle on Hulme Lane before being freed and transported to hospital with non-life threatening injuries.
