Michail Antonio put West Ham ahead on 18 minutes with a stunning overhead kick and it needed an equaliser from substitute Phil Foden early in the second half to earn Pep Guardiola's side a share of the points.
Manchester United and Chelsea later cancelled each other out in a goalless draw at Old Trafford while Crystal Palace went fifth thanks to a 2-1 win at bottom side Fulham.
Manchester City's faltering start continued as they dominated possession at Upton Park but could not make it count.
Antonio leapt acrobatically to score with an overhead kick for West Ham but when Foden, on as a second-half substitute, turned sharply to score the equaliser six minutes after the break, it looked like they would go on to win.
Guardiola sent on Kevin de Bruyne with just over 20 minutes left after his return from injury but the Belgian could not impact the game and West Ham held on for the draw.
It's the first time Guardiola has ever started a season as manager with less than 10 points from five games and City are now 12th, one spot behind West Ham.
"We started really well," Guardiola said. "We suffered in the last 10 or 15 minutes of the first half. In the second half, we were better and scored early. We had the chances we needed to win the game.
"I'm not a guy who can predict the future. We've already dropped seven points, which is a lot. But we've struggled a lot for many reasons. We have to take things game by game and see what happens."
The result left City still in the bottom half of the table, in 12th, one place behind West Ham.
At Old Trafford, United and Chelsea managed only four shots on target as they fought out a 0-0 draw.
Edouard Mendy made two great saves to deny Marcus Rashford while Chelsea might have had a penalty for what looked a clear foul by Harry Maguire on Cesar Azpilcueta.
But the two sides could not find the winner and Chelsea ended the day sixth, two points ahead of United, who stayed 15th.
Jairo Riedewald put Palace in front at Fulham with the first goal on eight minutes and Wilfried Zaha added a second just after the hour mark.
Fulham's Aboubakar Kamara was sent off two minutes from time for a challenge on Eberechi Eze and though Tom Cairney pulled one back in injury-time, Fulham remain bottom of the table with just one point from six matches.
