Detective Superintendent Michael Rowan has been listed to appear in the Downing Centre Court in Sydney on January 21.
He is charged with the failure to produce a document etc required by summons etc, and knowingly give false or misleading evidence at examination.
In 2019, the Law Enforcement Conduct Commission released the findings of an investigation into Superintendent Rowan.
It concluded that he had lied about the nature of his relationship with a lower-ranked officer and had engaged in misconduct in nine of the 14 matters investigated.
The report found that Superintendent Rowan had not declared his close personal relationship with a senior constable between 2016 and 2018.
The Law Enforcement Conduct Commission recommended that the Director of Public Prosecutions consider laying charges.
Superintendent Rowan's legal representatives submitted to the commission that he "maintains his denials as to lying" and "he gave no evidence which was false".
Superintendent Rowan played a leading role in the investigation into the murder of Leeton schoolteacher Stephanie Scott, which saw Vincent Standford sentenced to life in prison.
He was also in charge of the Griffith Local Area Command in 2013 when forensic officers conducted a large-scale search of farm land at Hay in a bid to locate the remains of Donald Mackay.
