Mr Griffiths replaced Margaret Bennett in the top job in August last year after being in the deputy role for the previous six years.
Health service board chairman Jonathan Green confirmed the resignation yesterday.
"Tim has made an outstanding contribution to the executive leadership of Northeast Health Wangaratta as our deputy CEO over the last six years, and more recently as CEO," Mr Green said.
"We will be very sorry to farewell him.
"Tim was not planning or anticipating leaving us at this point, but an opportunity has presented itself which enables Tim to take on a senior role in the NSW Health system as the executive director operations deputy chief executive of Southern NSW Health."

Fiona Shanks has been appointed to the interim CEO position.
"Fiona is very well placed to provide executive continuity and strong leadership, and her recent experience as the deputy CEO means that she is well briefed on the key areas of focus for our organisation," Mr Green said.
Mr Griffiths ascended to the top job at Northeast Health Wangaratta at the height of the COVID-19 pandemic response.
"I've got enormous respect for the job Tim has performed at Northeast Health," Ovens Valley MP Tim McCurdy said.
