A Toyota LandCruiser utility and Isuzu wagon collided in the northbound lanes at Table Top about 9.15am on Friday.
The force of the crash tipped over the caravan and left both cars hanging by two wheels.
A NSW Ambulance spokesman said there appeared to be six people involved, including four children.

One driver had a laceration to their head and another patient had back pain.
Both were taken to Albury hospital in a stable condition with non-life threatening injuries.
The other four people were assessed by some of the five ambulance crews on scene but didn't require hospital treatment.
The crash, which occurred a short distance past the speed monitoring cameras near Tynan Road, led to minor delays.
Heavy haulage vehicles were needed to remove the wreckage.
