The Crows looked to be drifting to defeat at Lavington Sports Ground, having mustered just 5/74 in 16 overs replying to the home side's 5/129.
East Albury needed an unlikely 56 off 24 balls at that point but when Luke Brunckhorst took Liam Scammell for 18 runs in the next over, the impossible became merely improbable.
The odds lengthened again when Brunckhorst skied a catch and departed for an entertaining 34 off 26 balls but still the visitors failed to read the script.
Kris Milosta, unwell at the start of the day and batting down the order for the first time this season, monstered three sixes and left East Albury needing just eight from the last over.
However, the pendulum swung again as Patties skipper Dean Nicholson conceded just one run from his first five deliveries, leaving the Crows on the verge of defeat.
But Milosta stunned the men in green by smashing the final ball of the match for six, levelling the scores and forcing the tie-breaker.
Kori Stevenson, having earlier picked up 3-11, bowled first in the Super Over and kept East Albury to seven, with Milosta and Brunckhorst unable to find the boundary.
It fell to Cameron White to bowl the Crows to victory, having Scammell caught second ball and then trapping Mitch O'Brien lbw to finish proceedings in the most dramatic fashion.
Remarkably, it's the second week running East Albury have prevailed in a Super Over after edging past Wodonga Raiders.
"I'm so happy for two blokes who have given so much to our club for a long period of time in Kris Milosta and Cameron White," Crows captain Brett Davies said.
"They're the lifeblood of East Albury, I love them to bits and they both came up trumps today. It was awesome.
"I never gave up (towards the end of our run-chase). One of our strengths is that we bat very deep.
"Kris-o wasn't well this morning but when the chips are down, the cream rises to the top and I'm so super proud of him.
"He's a hard worker, he's a pain in the arse sometimes too, don't worry, but today he came up with the goods.
"He's just full of ticker. Some blokes have the fabric, a bit of blue-collar stuff and he's full of that."
Milosta blasted 28 off 10 balls but even he admitted East Albury were staring down the barrel when he came to the crease.
"To be behind by so much with so little to go, it made the emotions so much greater," he said.
"I just bat the way I bat and it came off today.
"A few came out of the middle and I got lucky.
"It's very different (what I did today) to batting one or two, the ball's not doing as much and I found it quite easy, to be honest, to hit through the line of the ball.
"They were very chatty in that last over, up and about. It sort of felt as if they'd won the game. Deano bowled well in that last over but I got lucky on the last ball and hit a six.
"He was bowling really full so I thought I'd have to get right back as far as I can and try to get the length.
"I picked it up and got into the wind.
"It was the best feeling. I was smiling from ear to ear.
"When the umpire's finger went up in the Super Over, I don't reckon I've run faster today. I just charged in.
"To see all the boys so happy, it's only our second win so it's indescribable."
Nicholson was shattered afterwards but was quick to give credit to the opposition.
"East Albury played a really good game, they played good cricket late," he said.
"It's hard to criticise or analyse because it was a good game of cricket.
"There was a four or five over period we could have done better when we were batting and the same when we were bowling.
"I guess T20's such a short format, your margin for error is so small.
"Losing a game like this does hurt."
