Demarte was involved in an off the ball clash during the first quarter with Chiltern recruit Brad Hibberson in the Tigers opening round clash on April 11 and underwent surgery the following Monday.
He suffered a fractured cheekbone, eye socket and nose alongside multiple fractures to the surrounding areas.
Demarte had a metal plate alongside mesh inserted into his face and spent several days in hospital after the surgery.
The matter was referred to the league’s investigation officer, Enzo Bevacqua, on Friday.
He has 21 working days to complete his investigation.
Bevacqua will speak to players, coaches and club officials before presenting the league with his findings.
Barnawartha has submitted a $500 application fee for the probe.
Demarte, 34, is in the twilight of his career and has represented the league on many occasions.
He coached Barnawartha from 2010-12 before being replaced by Mark Sarau the following season.
The reliable defender was part of the Tigers’ 2013 premiership when they came from the elimination final to snatch the flag in stunning fashion.
Despite battling a crippling injury toll, the Tigers remain one of three unbeaten sides after two rounds and sit third on the ladder.
All sides with the exception of Beechworth and Rutherglen have the bye this weekend for Anzac Day.
The Tigers will face raging flag favourite Kiewa Sandy Creek the following week at home.
