Border aged services organisation Age Concern has joined forces with regional community services provider CareWest to continue to provide a range of essential aged care services to the community.
It provides services and activities for frail older people or people with disabilities and their carers. Support provided includes:
- Home care packages;
- Domestic assistance and personal care;
- Home modifications and maintenance;
- Volunteering opportunities;
- Case management; and
- Social support for individuals and groups.
The services it provides are funded by a range of government initiatives and each of these has particular eligibility criteria.
Entry to each service is via an assessment process and operates under guidelines that acknowledge and protect the participants:
- Rights to be represented in service planning;
- Confidentiality;
- Access to information, including their own records; and
- The right to make a complaint.
There is also a range of activities for community members including exercise programs.
CareWest is a regionally based organisation that delivers a number of community services.
These include aged, carer, child and family and disability services.
