North-East Health, Alpine Health and North-East General Practice Service are among the services receiving grants of more than $80,000 to invest in pulmonary and cardiac rehabilitation clinics and improve quality of chronic illness care.
Murray Primary Health Network executive director Penny Wilkinson hoped capacity in the region would be boosted.
“Our role is to help strengthen the health system in our region and to work towards equity and best practice models of care in the management of chronic illness,” she said.
“We know that respiratory and cardiovascular diseases affect more than 40,000 people in our region.
“Combined, they cause more than 4000 avoidable hospital admissions each year, and more than 200 avoidable deaths.”
North-East General Practice Service will establish a chronic obstructive pulmonary disorder clinic within the practice and Alpine Health will establish a chronic disease service in Alpine Shire area.
North-East Health is implementing the Central Hume Chronic Care connections program.
