The Albury resident, 83, had been a passenger in a silver Honda Jazz, which had stopped at the Hume Highway off ramp near High Street about 1.45pm.
The vehicle was rammed by a four-wheel-drive driven by a man who had recently finished work.
Senior Constable Mathew Higgs said the driver of the four-wheel-drive may have had a microsleep.
“It’s possible that fatigue was an issue,” he said.
“The passenger was taken to Albury hospital.
“It’s an important reminder that if you are fatigued, you should rest before driving.
“A microsleep can kill.”
The rear of the Honda was badly damaged but the four-wheel-drive had little damage.
A man has also walked away with only minor injuries after rolling his car several times at Sandy Creek.
Emergency services were called to Lockharts Gap Road about 9.40pm Thursday.
The 39-year-old had been driving a Nissan Navara utility when he lost control of the vehicle, and rolled it several times.
He suffered minor injuries and was taken to Albury hospital by ambulance.
Leading Senior Constable Jim Smeeton said police were investigating.
While it was too early to determine a cause, he said the man had been lucky to escape serious injury.
The car was written off and was towed to Wodonga.
