Little separated the two sides in the first-half before injuries to two key Swans proved telling in the second-half.
Chiltern's Janita Stephens suffered a serious knee injury and will undergo scans this week to determine the extent of the damage.
Hawks coach Kath Evans said the injuries proved to be the turning point in the hard-fought match.
"They copped a couple of bad injuries which wasn't great," Evans said.
"The really pressured us that first-quarter and we were only a goal up at the first break.
"At half-time it was still tight and it was only when they got the injuries were we able to get on top."
The Hawks have now strung together seven consecutive wins after losing to Thurgoona in the opening round.
