Ashley John Tyrell will face the Wodonga Magistrates Court in January after allegedly striking another footballer, Kelvin Wallace, leaving him with a broken jaw in several places.
Tyrell, a Federal player, was reported by a field umpire immediately after the incident, which occurred with only a few minutes left in the final quarter.
Mr Wallace required surgery after the game, which was played at the Tallangatta Valley Recreation Reserve on September 8.
The Bullioh player had been left with a tingling sensation in his neck.
Officers investigated the incident and recently laid charges.
The matter will be heard before the court on January 22.
Mr Wallace works as a self employed carpenter and had to take several weeks off work following surgery.
His jaw had been wired shut for nearly a month due to the damage.
He also had to make several visits to a dentist due to the injuries to his teeth, with follow-up visits made to the surgeon.
Photographs taken at the time showed extensive damage to his teeth.
An X-ray depicted deep fractures in his jaw.
Mr Wallace plans to continue playing football next season.
