The Blues cleaned up Federal by 16-goals on their home turf a Cudgewa Recreation Reserve in challenging weather conditions.
It capped off what has been a sensational season for the Blues after finishing the season minor premiers and only conceding one game in finals to the Swans.
Cudgewa goal attack Sophie Greenhill's efforts didn't go unnoticed with the star shooter claiming the medal for best on court in the grand final clash, scoring 24 goals.
It comes in the same season Greenhill racked up a whopping 73 goals in one game.
Greenhill said she was stoked with her side's achievement to go back-to-back.
"It was pretty hard to go undefeated all year and then lose in that second semi, but it was probably the touch up we needed," Greenhill said.
"We just all had an amazing season.
"I can't really take any credit because there's a 50-50 chance with how many goals you can get.
"Not many people on court get the chance to shoot goals.

"I wouldn't be able to do what I do without the rest of the team.
"We were lucky we were up by so much in the last quarter, because there weren't many goals scored with the conditions."
Joanna Beirs was instrumental for the Blues in centre with superb assistance from wing-attack Ellie Ross, who also played a full game in B-grade beforehand.
Anna Beirs also capped off a brilliant season in defence after making the switch from goal shooter this year.
Cudgewa coach Julianne Reiners credited her side for their versatility.
"It shows that our players are really adaptable," Reiners said.
"It hasn't been the last week that we've trained to get here, it's been the whole season.
"Coming home for a win today has been the icing on the cake."
League best and fairest and Federal co-coach Sophie Pattison and Blues defender Megan Nankervis matched-up well in the midcourt, while Sallie Findlay also had a good game for the Swans.
Pattison said she's proud of her team's efforts this season, despite not being able to come away with the flag.
"They played better than us for four quarters and that's just how it works unfortunately," Pattison said.
"Had the grand final been two weeks ago, then that would have been our game.
"It's great to even get to a grand final, so that's a positive in itself."
