A woman in her 80s, who had been a resident at the Wyoming Residential Aged Care Facility in Summer Hill, has died at Royal Prince Alfred Hospital.
NSW Health's Dr Jeremy McAnulty said there had been positive sewage detections in Dubbo and Mudgee.
"Where we see sewage detections in communities without known cases, we have to be vigilant," he said.
Dr McAnulty said NSW Health was imposing new restrictions on 12 suburbs in Penrith, where the "front of the epidemic has moved into".
NSW CrimeStoppers continues to receive around 1000 calls a day with complaints of people breaching health others.
NSW Premier Gladys Berejiklian wants to hit six million jabs by the end of August.
"That will provide us an opportunity to consider what life looks like beyond September," she said.
"Vaccination is key to reducing the spread.
"As horrible as every death is, there still hasn't been a death where people have received both doses."
South of the Murray River, Victoria has recorded 11 new cases.
The 11 new locally-acquired cases are all linked to previously reported cases and have not been in quarantine throughout their infectious period.
In Victoria, 17,360 vaccine doses were administered.
Meanwhile, the South-east Queensland lockdown is being lifted, but Cairns and Yarrabah will enter a three-day lockdown.

Health Minister Greg Hunt, updating on approval processes for a new treatment for COVID, said the situation in Queensland was "promising", while there was "hope" in Victoria and "progress in NSW".
"They are containing (the outbreak), but we know there are challenges," Mr Hunt said.
Mr Hunt has announced $17.7 million in additional mental health measures.
"The lockdowns can be difficult and gruelling ... mental health support is absolutely critical," he said.
"Head to help clinics will be extended to June 30 2022 in Victoria."
Sotrovimab won't be used as treatment until the Therapeutic Goods Administration has approved it.
It is an antibody that will be mostly aimed at people not vaccinated and who are at the highest risk of severe disease, early in the treatment course for COVID-19.
