O'Connell polled 25 votes from 13 games played to finish eight points clear of Kiewa-Sandy Creek runner-up Georgie Attree during the TDNA's virtual count.
She's believed to be the first to claim an A-grade medal in all three leagues.
The talented 37-year-old midcourt player became Corowa-Rutherglen's first Toni Wilson medallist in 2010, after finishing second to Albury's Adelle Gormly the year prior.
During her three years in the Hume League, she claimed the association's top honour in her first season and went on to be runner-up in 2017 and 2018.
Her time at Billabong Crows was further sweetened after she led the club to their first top grade flag before stepping down as coach.
Returning to Corowa-Rutherglen in 2019, O'Connell went back-to-back to claim another premiership with the Roos' B-grade side.

Having already achieved so much in her outstanding netball career, O'Connell said the honour was just as exciting as it was the first time.
"I was still reeling from being named in the team of the year, because that's voted by your peers," she said.
"It's a huge achievement to be recognised and respected by those that you compete against."
She confirmed she'll be remaining at the Roos next season.
"My desire is to now win a premiership in the Tallangatta League," she said.
"That would be the real trifecta."
Runner-up Attree shot 119 goals in her first official season as a Hawk.
Other leaders were Eliza Abbott, Zoe Rae (15), Alana Sutton (14), Ava Kennett, Hannah Dwyer, Kirsty Baude and Keiran De Koeyer (13).
