The family medical centre on Griffith Road has confirmed the final day of operation will be Friday, January 24, with all five doctors leaving.
The notice said the doctors would be leaving the clinic at the end of January due to "business reasons" but couldn't confirm what would happen to the clinic.
IN OTHER NEWS:
The practice confirmed the closure on the weekend with advertisement in Saturday's The Border Mail.
Practice principal Nick Nourse couldn't be contacted for comment on Sunday, but the statement said he was moving out of the region.
"Patients' medical records remain confidential and will be kept in accordance with the relevant legislation," the statement said.
"We will require individual patient consent for medical records to be released to another health care provider.
"To arrange for a transfer of your health records, or to discuss any other matters please contact the clinic on 6025 2777 prior to January 24."
One of the longest-serving doctors, Alison Palmer, who has been seeing patients from the clinic since 1998, won't be returning to general practice.
"Dr Joanne Tong is leaving the region, Dr M.Masab Saleem will be practicing from The Doctors at Lavington and Dr Lwin Thu is commencing maternity leave in February 2020," the statement said.
The late Brendan Pattison, who died in August 2015, was a much-loved family doctor at the clinic for more than 30 years.
He owned the clinic up until his retirement a few months before his death.
It is not known what will happen to the employment of four reception staff and three nurses.
The clinic had many long-term patients, many of them having seen doctors there since it opened in the late 1970s, and hadn't been taking new patients for a few years.
"We would like to thank all the people who have been involved with the Lavington Clinic over the years," the statement said.
"We wish you all the best."
The future of the building is unclear.
The clinic can be contacted after its closure in writing to PO Box 503, Lavington or by emailing myhealthrecord@lavingtonclinic.com.au.
